Öğe An Empirical Study of Verb Use as Indicator of Emotional Access in Therapeutic Discourse(Educational Publishing Foundation-American Psychological Assoc, 2017) Halfon, Sibel; Fisek, Guler; Cavdar, AlevVerbs are the primary linguistic vehicles connecting one to his or her affective bodily core. Previous studies have shown that an increase in patient's verb repetitions in psychoanalysis is indicative of sensory affective arousal that cannot be narrated in words. If the treatment is operating effectively, the patient can use the therapeutic medium to transform the raw affect into a coherent emotional narrative. Using this premise, this study empirically investigated the effectiveness of a psychoanalytic treatment, whose success has been controversial in the past. Using a single-case design with primarily quantitative linguistic methods, measures of verb repetitions, affect dictionaries, and a computerized measure of Referential Activity (a measure of imagistic emotional language) were applied to verbatim recordings of 30 psychoanalytic sessions representing different years of treatment. Results showed that the verb repetition measure was able to differentiate significantly the sessions in terms of level of affective arousal, and there was a significant decrease in mean verb repetitions across the year. Further quantitative and clinical analyses supported that an increase in the repetition of action verbs and affective verbs was associated with intense affective sensory arousal that disrupts the patient's ability to think where as an increase in stative verb repetitions helped the patient organize a meaningful emotional narrative. Results suggest that a systematic study of verbs can reveal crucial information about patient's affective states and provide pointers for the clinicians in terms the kinds of interventions needed according to the patient's linguistic choices.Öğe Assessing Affective Dimensions of Play in Psychodynamic Child Psychotherapy via Text Analysis(Springer International Publishing Ag, 2016) Halfon, Sibel; Oktay, Eda Aydin; Salah, Albert AliAssessment of emotional expressions of young children during clinical work is an important, yet arduous task. Especially in natural play scenarios, there are not many constraints on the behavior of the children, and the expression palette is rich. There are many approaches developed for the automatic analysis of affect, particularly from facial expressions, paralinguistic features of the voice, as well as from the myriads of non-verbal signals emitted during interactions. In this work, we describe a tool that analyzes verbal interactions of children during play therapy. Our approach uses natural language processing techniques and tailors a generic affect analysis framework to the psychotherapy domain, automatically annotating spoken sentences on valence and arousal dimensions. We work with Turkish texts, for which there are far less natural language processing resources than English, and our approach illustrates how to rapidly develop such a system for non-English languages. We evaluate our approach with longitudinal psychotherapy data, collected and annotated over a one year period, and show that our system produces good results in line with professional clinicians' assessments.Öğe Çocuk ve Ergen Ruh Sağlığı ve Hastalıkları Alanında Bir Klinik Gidiş Değerlendirme Ölçeği Türkçe Formu’nun (HoNOSCA-TR) psikometrik özellikleri(2020) Kara, Demet; Çavdar, Alev; Halfon, SibelÇocuk ve Ergen Ruh Sağlığı ve Hastalıkları Alanında Bir Klinik Gidiş Değerlendirme Ölçeği(Health of the Nation Outcome Scales for Children and Adolescents (HoNOSCA) İngiltere’de ruh sağlığı alanındasonuç tarama amacıyla klinisyenler tarafından doldurulmak üzere geliştirilmiştir. Ölçeğin geçerliği ve güvenirliğiTürkiye’de küçük bir örneklem üzerinde daha önce değerlendirilmiştir. Bu çalışmanın amacı HoNOSCA’nınTürkçe uyarlamasının (HoNOSCA-TR) geçerlik ve güvenirliğinin daha geniş bir örneklem ile saptanmasıdır.İstanbul Bilgi Üniversitesi Psikolojik Danışmanlık Merkezi’nde psikoterapi gören 168 çocuğun aile, öğretmen veterapistlerinden psikoterapinin başında ve sonunda HoNOSCA-TR’nin yanı sıra Çocuk ve Genç Davranış DeğerlendirmeÖlçeği, Öğretmen Bilgi Formu ve Çocukların Global İşlevsellik Skalası’nı doldurmaları istenmiştir.HoNOSCA-TR’nin faktör yapısı açımlayıcı ve doğrulayıcı faktör analizi ile incelendiğinde faktör örüntülerininözgün ölçekteki örüntüyle örtüşmediği; ancak davranış ve bilişsel sorunlar, algısal ve fiziksel bozulmalar, duygusalve sosyal sorunlar olmak üzere üç faktöre ayrıştığı ve kavramsal olarak tutarlı, istatistiki olarak anlamlı bir yapıoluşturduğu görülmüştür. Ancak, faktörlerin iç tutarlılıkları zayıftır. HoNOSCA-TR diğer değerlendirme ölçekleriile anlamlı istatistiki ilişki göstermiş ve danışanları tanıya göre ayrıştırmıştır. Ayrıca kodlayıcılar arası güvenirlikve test-tekrar test güvenirliği yeterli düzeyde bulunmuştur. Danışanların psikoterapi öncesi ve sonrası puanlarındabulunan anlamlı farklılaşma HoNOSCA-TR’nin değişime hassasiyetine işaret etmiştir. Bu çalışma, HoNOSCA-TRhakkında daha önce yapılan geçerlik ve güvenirlik çalışmasını desteklemektedir. Mentalization-based interventions have the potential to create a safe relationship, which makes further mentalizing interventions possible. However, to date, no study has examined the bidirectional relationship between these variables in child psychotherapy. In an evidence-based case study design, psychodynamic therapy processes of two Turkish children (age: 9 and 10 years) who presented with social withdrawal problems were compared. All their sessions (34 sessions from Case No. 1 and 31 from Case No. 2) were coded using the Therapy Process Observational Coding System-Alliance Scale and Mentalization-Based Treatment for Children Adherence Scale. Outcome scales pertaining to symptoms, attachment, and mentalization were administered at baseline and at termination. Time-series Granger Causality tests revealed that in the case with clinically significant outcome, mentalization techniques predicted therapeutic alliance in the subsequent sessions, which predicted the use of further mentalization interventions. However, this relationship was not supported in the case with no significant improvement. Selected sessions were clinically analyzed with verbatim session vignettes. Our findings indicate that mentalization techniques in child therapy are closely related to the therapeutic relationship.Öğe Ebeveyn Gelişim Görüşmesi Üzerinden Yapılan Yansıtıcı İşleyiş Kodlama Sisteminin Psikometrik Özellikleri(2021) Halfon, Sibel; Besiroglu, BurcuEbeveynin yansıtıcı işleyişi kendisi, çocuğu ve ikisi arasındaki ilişkiye dair zihin durumları hakkında düşünebilme, bunları birbirine bağlayabilme ve etkilerini tahmin etme becerisidir. Bu çalışmada, Ebeveyn Gelişim Görüşmesi üzerinden yapılan yansıtıcı işleyiş kodlama sisteminin psikometrik özellikleri incelenmiştir. Çalışmaya bir üniversite kliniğinde psikoterapi hizmetine başvurmuş 115 anne (Ortyaş = 36.01 yaş, SS = 4.96) ve çocukları (Ortyaş = 6.99 yaş, SS = 2.09, %41.7 kız) katılmıştır. Ebeveynlerle Ebeveyn Gelişim Görüşmesi yapılmış, görüşme yansıtıcı işleyiş kapasitesi kodlama sistemi üzerinden değerlendirilmiştir. Ayrıca, ebeveynlerin sosyo-demografik bilgileri ve bağlanma stili ile çocukların duygu düzenlemesi, ifade edici dil becerisi, çocukluk çağı deneyimleri ve davranış sorunları ile ilgili bilgiler toplanmıştır. Açımlayıcı ve Doğrulayıcı Faktör analizi sonuçları üç faktörlü bir yapı ortaya çıkarmış ve her faktörün iç tutarlılık değerlerinin yeterli olduğu görülmüştür. Bunlar kavramsal olarak, çocuk- (ebeveynin çocuğunun duygularını yansıtıcı işleyiş kapasitesi), ilişki- (ebeveynin anne-çocuk arasındaki ilişkiyle ilgili yansıtıcı işleyiş kapasitesi) ve kendi-odaklı (ebeveynin kendi duygularını yansıtıcı işleyiş kapasitesi) faktörler şeklinde isimlendirilmiştir. Çocuk-odaklı yansıtıcı işleyiş kapasitesi sosyoekonomik durum ile pozitif yönde ilişkilidir. İlişkiodaklı yansıtıcı işleyiş kapasitesi ebeveynin kaçınmacı bağlanma stili, eğitim seviyesi, sosyoekonomik durumu, annenin sahip olduğu çocuk sayısı ve çocuğun olumlu ve olumsuz deneyimleri ile ilişkilidir. Kendi-odaklı yansıtıcı işleyiş kapasitesi ise annenin sahip olduğu çocuk sayısı, çocuğun ifade edici dil becerisi, duygu düzenlemesi ve dışsallaştırma sorunlarıyla ilişkilidir. Ayrıca çocukların aile, öğretmen ve terapistlerinden psikoterapi sürecinin başında Çocuk ve Genç Davranış Değerlendirme Ölçeği, Öğretmen Bilgi Formu ve Çocuklar için Genel Değerlendirme Ölçeği’ni doldurmaları istenmiştir. GDTİÖ’nün faktör yapısı incelendiğinde, bu çalışmadaki yapının özgün yapıdan farklı, fakat daha sonra yapılan faktör analizi çalışmalarında ortaya çıkan faktör yapısıyla benzer olduğu, olumlu ve olumsuz ittifak olarak ayrıştığı görülmüştür. Bütün ölçeğin ve olumlu ittifak faktörünün iç tutarlılık katsayılarının yeterli düzeyde olduğu, ancak olumsuz ittifak faktörünün iç tutarlılığının düşük olduğu bulunmuştur. Ancak, ölçeğin psikometrik özelliklerinin farklı örneklem ve terapi yönelimleri ile sınanması gerekmektedir.Öğe Interaction structures as predictors of outcome in a naturalistic study of psychodynamic child psychotherapy(Routledge Journals, Taylor & Francis Ltd, 2020) Halfon, Sibel; Goodman, Geoff; Bulut, PelinsuObjective: The first aim of this study was to identify interaction structures (IS), which refer to clusters of items characterizing the course of psychotherapy in terms of reciprocal interaction patterns between the therapist and the child, secondly to assess their trends over the course of treatment, and finally investigate which IS predict outcome in long-term psychodynamic child psychotherapy. Method: The sample included 52 children with externalizing and internalizing problems. 192 sessions were rated with the use of the Child Psychotherapy Q-Set (CPQ). Outcome was assessed with the Child Behavior Checklist (CBCL) Total Problems, and Children's Global Assessment Scale (CGAS). Results: A principal components analysis resulted in four IS, theoretically named Therapeutic Alliance, Children's Emotion Expression, Child -Centered Technique and Psychodynamic Technique. Multilevel Modeling (MLM) analyses indicated significant linear growth on Psychodynamic Technique. Multiple regression analyses indicated that Psychodynamic Technique positively predicted changes on CBCL Total Problems and CGAS. In contrast, Child-Centered technique negatively predicted change on CBCL Total Problems, however follow up interaction analyses showed that externalizing children who received more Child-Centered technique showed more improvement on this scale. Discussion: Findings point to empirically derived components of psychodynamic child psychotherapy and provide preliminary answers about which aspects may facilitate change.Öğe Interaction Structures in Psychodynamic Psychotherapy for Adolescents(Mdpi, 2021) Can, Baris; Halfon, SibelDespite advances in psychotherapy research showing an evidence-base for psychodynamic psychotherapy (PDT) in adolescents, developmentally specific treatment characteristics are under-researched. We aimed to identify interaction structures (IS: reciprocal patterns of in-session interactions involving therapist interventions, patient behaviors, and the therapeutic relationship) and assess associations between IS and outcome. The study cohort comprised 43 adolescents (M-age = 13.02 years) with nonclinical, internalizing, and comorbid internalizing-externalizing problems in PDT. A total of 123 sessions from different treatment phases were rated based on the Adolescent Psychotherapy Q-Set (APQ). Outcome was assessed with the Brief Problem Monitor-Youth (BPM-Y) administered repeatedly over the treatment course. Principal component analysis of APQ items resulted in five IS, named Negative Therapeutic Alliance, Demanding Patient, Accommodating Therapist, Emotionally Distant Resistant Patient, Inexpressive Patient, Inviting Therapist, and Exploratory Psychodynamic Technique (EPT). Multilevel modeling analyses with Bayesian Markov chain Monte Carlo (MCMC) estimations indicated a two-way interaction effect between EPT and problem levels at baseline such that patients with lower problems at baseline showed good outcome in the context of EPT, whereas an inverse relationship was found for patients with higher problems. Findings provide empirical evidence for characteristic components of PDT for adolescents and preliminary answers about who benefits from psychodynamic techniques.Öğe Introduction to the Section: Widening the Scope of Psychodynamic Child and Adolescent Psychotherapy Research: Recent Applications and New Directions(Routledge Journals, Taylor & Francis Ltd, 2022) Halfon, SibelRecent developments in psychodynamic work with children and adolescents have made empirical research a higher priority in the field. In order to support the evidence-base of psychodynamic child and adolescent psychotherapy, more research is needed to identify the underlying therapeutic processes that make psychodynamic psychotherapy effective for different kinds of children and adolescents and contribute to the literature on therapeutic mechanisms of change. Drawing on the expertise of a range of contributors, this special section describes recent child and adolescent psychodynamic psychotherapy research illustrating a range of quantitative and qualitative methodologies that have been developed to investigate different clinical phenomena, processes of psychotherapy and outcome. These papers will serve as a vital resource helping researchers and clinicians develop clinically relevant research agendas to improve clinical practice and care with children and adolescents.Öğe Literary and Analytic Transformations of Trauma: Repetition, Revision and Rebirth in Two Stories of Raymond Carver(Educational Publishing Foundation-American Psychological Assoc, 2016) Halfon, Sibel; Weinstein, LissaA linguistic analysis of two related short stories written by Raymond Carver, The Bath (1981) and A Small, Good Thing (1983), highlights the differences in their narration of the same traumatic event-the sudden death of a child. In The Bath, the monotony and repetition of the linguistic structures and the lack of descriptive nouns preserve the depicted trauma in its original raw form. In contrast, in A Small, Good Thing the emphasis on detailed descriptions, and the varied use of language transforms the brutally realistic trauma into a deeply moving and affectively rich experience. A parallel can be drawn to the psychoanalytic process, which aims to move the patient from fixed traumatic repetitions toward sublimated forms of representation within the transference. Adele Tutter (2009, 2011) has examined different versions of Carver's stories in relation to Carver's autobiography and discussed how he projected himself in the narrative. Building on Tutter's work, this article will attempt a systematic comparison of the language of these 2 stories, discussing the trajectory from traumatic repetitions to symbolization in psychoanalytic theory.Öğe Mentalization and the growth of symbolic play and affect regulation in psychodynamic therapy for children with behavioral problems(Routledge Journals, Taylor & Francis Ltd, 2019) Halfon, Sibel; Bulut, PelinsuObjective: Children with behavioral problems often have problems with symbolic play organization, specifically with the regulation of negative affect and its representation. One of the aims of psychodynamic therapy with these children is enhancing their symbolic and mentalizing capacities in play. This study investigated the associations between promoting mentalization, and the growth of symbolic play and affect regulation. Method: The sample included 48 children with behavioral problems, who underwent long-term psychodynamic treatment informed with mentalization principles, with good outcome. Three hundred twenty-nine sessions were coded for children's symbolic play and affect regulation strategies, and each treatment was rated for adherence to mentalizing principles. Results: Hierarchical linear modeling showed quadratic growth of variables, wherein symbolic play initially increased followed by a deceleration in change, whereas affect regulation initially decreased followed by an increase. Adherence to mentalizing principles was associated with both symbolic play and affect regulation. A two-way interaction between time in treatment and adherence to mentalization showed that in high adherent treatments, affect regulation increased towards the end of treatment, whereas in low adherent treatments, there was no change. Conclusion: Results show support for the association between adherence to mentalization and growth of affect regulation and symbolic play in psychodynamic therapy.Öğe Mentalization, Session-to-Session Negative Emotion Expression, Symbolic Play, and Affect Regulation in Psychodynamic Child Psychotherapy(Amer Psychological Assoc, Div Psychotherapy, 2019) Halfon, Sibel; Yilmaz, Meltem; Cavdar, AlevInternalizing and externalizing problems have been related to negative emotionality, and affect regulation deficits in several studies. Certain psychodynamic models of treatment use children's symbolic play activity as a medium to mentalize negative emotions in order to develop children's affect regulation. However, the complex associations among these constructs and their associations with outcome have not been examined. This study aimed to investigate, first. whether average mentalization practices in treatment, as well as session-to-session expression of negative emotions and symbolic play predict gains in affect regulation and. second, whether changes in these variables are associated with clinically significant change in symptoms and global function. The sample included 40 outpatient children, who underwent long-term psychodynamic treatment. Nine hundred seventy-five sessions were coded for children's symbolic play, affect expression (anger and dysphoric affect). and affect regulation characteristics, and each treatment was scored for average adherence to mentalizing principles. Time Series Panel Analyses (TSPA) indicated session-to session use of symbolic play predicted gains in affect regulation. A significant 2-way interaction indicated that dysphoric affect expression in high mentalization adherent treatments was associated with higher affect regulation than in low adherent treatments. Partial correlation analyses indicated that mentalization adherence in treatment was associated with symptomatic improvement at trend level of significance, and changes in affect regulation and symbolic play were significantly associated with gains in global function. Findings point to the importance of use of symbolic play, and dysphoric affect expression in the context of mentalization practices for gains in affect regulation and outcome.Öğe Monitoring Nonlinear Dynamics of Change in a Single Case of Psychodynamic Play Therapy(Soc Chaos Theory Psychology & Life Sciences, 2019) Halfon, Sibel; Cavdar, Alev; Paoloni, Giulia; Andreassi, Silvia; Giuliani, Alessandro; Orsucci, Franco F.; de Felice, GiulioA systematic single case study with a mixed quantitative and qualitative methodology was conducted to investigate the nonlinear dynamics of change in play profiles of a child in psychodynamic play therapy. The first aim of the study was to cluster the different features of play characteristics (i.e., descriptive, cognitive, affective, and social characteristics of play, and the defensive strategies used in play) in order to construct the different play profiles of this child, and secondly to assess the transitions between profiles over the course of treatment. It was expected that there would be an increase in critical fluctuations during the transitions between the profiles and an increase in the variability of play profiles. Results showed that the child's play characteristics clustered into eight states and three attractors. The Markov Transition Matrix showed how play profiles evolved over time. Entropy analyses comparing the first and the second half of therapy showed an increase in variability. Qualitative analyses indicated the importance of expression of the child's underlying fear, and its integration with overt anger in the generation of the new play profiles. Method: The sample included 53 Turkish children: 41 with internalizing, externalizing and comorbid problems; 12 in the non-clinical range. We collected audio and video recordings of 148 sessions, which were manually transcribed. Independent raters coded children's expressions of pleasure, anger, sadness and anxiety using the Children's Play Therapy Instrument (CPTI). Automatic facial and linguistic affect analysis modalities were adapted, developed, and combined in a system that predicts affect. Statistical regression methods (linear and polynomial regression) and machine learning techniques (deep learning, support vector regression and extreme learning machine) were used for predicting CPTI affect dimensions. Results: Experimental results show significant associations between automated affect predictions and CPTI affect dimensions with small to medium effect sizes. Fusion of facial and linguistic features work best for pleasure predictions; however, for other affect predictions linguistic analyses outperform facial analyses. External validity analyses partially support anger and pleasure predictions. Discussion: The system enables retrieving affective expressions of children, but needs improvement for precision.Öğe Parental Reflective Function and Children's Attachment-Based Mental State Talk as Predictors of Outcome in Psychodynamic Child Psychotherapy(Amer Psychological Assoc, Div Psychotherapy, 2021) Halfon, Sibel; Besiroglu, BurcuMentalization, operationalized as reflective function, is defined as the capacity to understand behavior in terms of mental states. Mentalization can be self-focused (i.e., mentalizing that focuses on one's own thoughts and feelings) or other-focused (i.e., mentalizing that focuses on others' thoughts and feelings). Some studies in adult psychotherapy show the importance of patients' mentalization capacity for treatment outcome; however, this has not yet been investigated in psychodynamic child psychotherapy. This study aimed to investigate whether baseline parental reflective function (PRF) and children's mental state talk (MST) predicted changes in emotional and behavioral problems in psychodynamic child psychotherapy. The sample included 60 Turkish school-age children (M-age = 7.90, SD = 1.35, 43.3% girls) with internalizing (18.3%), externalizing (5%), and comorbid (56.7%) problems, and 20% of the children were in the nonclinical range. The mothers were interviewed using the Parent Development Interview, which was coded for PRF (self- and child-focused). Children were administered an attachment-based story stem task, coded for MST (self- and other-focused). The Brief Problem Monitor was administered every month over the course of treatment for a total of 366 sessions. Multilevel modeling analyses indicated that mothers' child-focused PRF and children's self-focused MST predicted changes in problem behaviors. Parents' mentalization about their children and children's mentalization about their own internal states could be predictors of treatment response in psychodynamic child psychotherapy.Öğe Psychodynamic Technique and Therapeutic Alliance in Prediction of Outcome in Psychodynamic Child Psychotherapy(Amer Psychological Assoc, 2021) Halfon, SibelObjective: This study is the first to investigate the effect of psychodynamic technique (PT), therapeutic alliance, and their interactions with outcome in psychodynamic child psychotherapy. Method: The sample comprised 79 Turkish children (mean age = 6.86 years, 38% girls) with discrete internalizing (22%), discrete externalizing (11%) and comorbid internalizing and externalizing (67%) problems. Independent raters coded 359 sessions from different phases of treatment using the Child Psychotherapy Process Q-Sort for PT and Therapy Process Observational Coding System-Alliance Scale. Problem-assessment measures were collected at intake and every 10th session in treatment using the Children's Behavior Checklist and Brief Problem Monitor. Results: Multilevel modeling analyses indicated that the PT and therapeutic alliance interacted such that more use of the PT in the context of high therapeutic affiance predicted less problem behaviors, whereas in the context of low therapeutic alliance PT predicted more problem behaviors. This relationship was moderated by problem comorbidity such that for children with comorbid problems, though a strong therapeutic alliance was indicated, an increase in PT use did not have a significant effect. Conclusion: Findings suggest that the use of psychodynamic interventions is indicated in the context of a strong therapeutic affiance, especially for children with noncomorbid problems. Psychodynamic interventions may have an adverse effect if a therapeutic affiance is not established. For children with comorbid problems, keeping the relationship strong is important.Öğe Session Adherence to Prototypical Psychotherapy Process as a Correlate of Outcome in a Naturalistic Study of Child Mentalization-Informed Psychodynamic Play Therapy(Educational Publishing Foundation-American Psychological Assoc, 2021) Goodman, Geoff; Halfon, SibelWhile the adult psychotherapy literature has made great strides to understand the relations between psychotherapy process and treatment outcome, the child psychotherapy literature lags far behind. The first aim of this study was to determine whether mentalization-informed psychodynamic play therapy (PDT) sessions adhered to therapeutic principles comprising this treatment model. The second aim was to assess whether session adherences to prototypical PDT and Reflective Function (RF) process using the Child Psychotherapy Q-Set (CPQ) were correlated with both problem behavior reduction and adaptive functioning after controlling for pretreatment scores. The third aim was to assess whether these correlations depended on problem behavior type (externalizing-only, internalizing-only, externalizing and internalizing), age, or gender. The sample included 95 children (56% male; M age = 6.80) categorized into these 3 types. Three-hundred seventy-nine sessions from different treatment phases were rated using the CPQ. Outcomes were assessed using the Child Behavior Checklist, Teacher Rating Form, Emotion Regulation Checklist, and Children's Global Assessment Scale. Sessions adhered to prototypical RF and PDT process. Session adherences to prototypical RF and cognitive-behavioral therapy (CBT) process were positively correlated with adaptive functioning. Among internalizers, session adherence to prototypical PDT process was negatively correlated with internalizing behavior, while among externalizers, session adherences to prototypical RF and CBT process were negatively correlated with externalizing behavior and emotion negativity. Session adherences to prototypical RF and CBT process were significantly higher for girls than for boys. Two different trajectories of success are proposed for children with externalizing problems.
